To add a couple more cents, I had this problem too, on a Samsung EVO 850 as well.
I had cloned it with the Samsung software and I now suspect that was the cause of our common problem.
Expanding the partition by 100Mb (to 200Mb) was enouth to sort the issue.

actually, i don't know why p2v conversion stops at 97% during your using vmware converter standalone, but i wanna to share another way for P2V, that is third party software AOMEI Backupper, it has a feature called universal restore which is available for p2p, p2v, v2p. really works well as i have tried it. (ps: if you sre server user, Server version is ok.)
the detail guide, you can refer to how to restore to server 2012 to different hardware.
Interesting. Does it really work for P2V? According to their forum"currently our software doesn't support dissimilar hardware restore (restore system to another computer)". Or is this obsolete info?
Even with full vCenter Administrator privileges, that would have full admin rights on that datastore, this did not work.
Please let me know if you have any other further thoughts or if the Log Bundle will help.

i need to convert xp with fat32. Anybody can help me? When i try to use Vm converter stand alone i get error message. I don't want to convert the system volume with ntfs.
I have used different version of Vm converter 3.0 and 5.0.

Try with Converter 6.0, if your FAT32 is a system volume, then you could succeed with conversion.
Otherwise you need at least one NTFS volume attached to the system in order to create snapshot, and don't need to convert file-system.

When I got the similar issue I was P2V all logical volumes separately. Try to P2V the C drive first if it will work, then do other drives to the separate VM and then remount vmdk to the original VM. Usually this method worked.

I got this message when trying to start vm after launching P2V process iwth VMware vCenter Converter
It seems that SATA disk type (from my source machine) are unsupported and Convert tools purpose is SCSCI type but I'm not sure ?
Any idea for this process ?

I use the latest version 6.0 of Converter.
I changed options of HD to IDE (ATA and IDE disks controllers are available in the physical machine configuration) before launching Converter again. I can power on the vm now. Thanks for your help.

I have been able to use p2v to copy from one instance of vsphere to another instance of vsphere within the same datacenter without any issues.Now i am trying to use p2v to copy from one datacenter to another. When i use the standalone converter it detects the target vsphere and shows me the directory structure including all the esx hosts and datastores. I can select the one i need and proceed from there. Everything seems to work fine until it actually tries to start the job. the helper is created and appears in the target datacenter's vsphere.. but then the process stops and it gets deleted. i can ping the target vsphere host from the originating vsphere host so i know network connectivity is there. SHOULD what i am trying to do be possible? does anyone have any ideas on what i should try differently.For the helper, I use an ip/gateway/subnet mask that is accessible from the ip of the parent VM.
Any advice would be greatly appreciated.

You will not be able to migrate to a destination host which is being managed by a vCenter. Either take the host out of the vCenter and migrate to it, or go through the vCenter.
Also telnet and make sure all the required ports are opened in both directions.
Yes, the HyperV from Win2k12+ is not supported but you can convert VMs like powered on machines, just provide an IP and credentials.
